Is this a new PC or existing PC and had CRM prior?
I'd try to uninstall CRM and SQL completely and then reinstall.
Complete uninstall (similar for 2011): mscrmonline.wordpress.com/.../full-uninstall-of-microsoft-crm-outlook-client
